Tubal Patency Test

A key diagnostic step in fertility care – procedure, costs, and chances at Kinderwunschzentrum Dresden

For nearly one in three couples with infertility, blocked or damaged fallopian tubes are the underlying cause. A tubal patency test is therefore an essential diagnostic tool in fertility medicine. It reveals whether the fallopian tubes are open, partially blocked, or closed – a crucial factor for natural conception. At Kinderwunschzentrum Dresden, we use advanced and gentle methods, such as hysterosalpingo-contrast sonography (HyCoSy), to provide clarity and guide your next steps.

What is a tubal patency test?

The tubal patency test checks whether the fallopian tubes are blocked or clear. If the tubes are open, the egg can reach the uterus for fertilisation. If blocked, pregnancy without medical assistance is unlikely.

When is the test recommended?

  • Trying to conceive for more than 12 months after other causes have been ruled out
  • Suspected adhesions after infections (e.g., chlamydia)
  • Suspected endometriosis
  • History of abdominal or pelvic surgery

Tubal patency test procedure

Methods at a glance

  • Hysterosalpingo-contrast sonography (HyCoSy): ultrasound with contrast medium – minimally invasive, no anesthesia.
  • Laparoscopy:. Laparoscopy is a minimally invasive procedure (keyhole surgery) used to examine the abdominal cavity. It is particularly recommended when endometriosis or adhesions are suspected, or when conditions such as cysts or fibroids require surgical evaluation. Laparoscopy plays an important role in diagnosis and is performed under general anesthesia. The procedure involves two to three very small incisions, only a few centimeters long. Adhesions around the ovaries can often be released during the same procedure. If surgery is needed, we work closely with experienced gynecological surgeons offering the full spectrum of operative care.

Step-by-step process (HyCoSy)

  1. A thin catheter (flexible tube) is placed in the uterus.
  2. Contrast medium is introduced.
  3. Ultrasound shows whether the contrast medium passes through the fallopian tubes.

The exam is performed on an outpatient basis.

Duration, discomfort, and recovery

  • Duration: usually 15-30 minutes
  • Pain/discomfort: often described as mild, similar to period cramps; pain medication can be offered
  • Sick leave: generally not required; taking a day to rest may be beneficial.

Chances after a tubal patency test

Pregnancy potential following the test

Studies suggest pregnancy chances may increase slightly in the months after the patency test since  “flushing” can clear small adhesions.

Meaning of results

  • Both tubes open: natural pregnancy possible
  • One tube blocked: pregnancy possible but less likely
  • Both tubes blocked: IVF or ICSI required for conception

Tubal patency test costs

How much does a tubal patency test cost?

  • HyCoSy (ultrasound with contrast): around €300

Insurance coverage

  • German statutory health insurance often covers costs if infertility is documented.
  • Private insurance coverage varies by contract.

Risks and side effects

  • Mild pain or cramping
  • Temporary spotting
  • Rarely: infection, mitigated by preventive antibiotics
  • Very rare: allergic reaction to contrast medium

Statistics and medical data

  • 30–40% of female infertility cases are related to fallopian tube dysfunction (source: German IVF Registry, DIR).
  • Women with open tubes have a 15–20% chance per month of natural pregnancy.
  • Blocked tubes typically require assisted reproduction such as IVF.

How much does a tubal patency test cost?

Around €300 for HyCoSy

Do I need time off work after the test?

Usually no. Only laparoscopic procedures require several days of recovery.
